Our online referral system is fast and easy! Simply follow the prompts to add patient/referral information, drag/drop to share x-rays and images, or leave any specific notations. Once the electronic referral is submitted, our office will receive this information immediately. Be sure to include the patient's phone number on the electronic form and the referral will be texted directly to your patient where they can easily access their referral information!

You may also download/print our referral slip and send the completed form to admin@abqimplants.net. We appreciate your trust in our team and look forward to providing exceptional care to your patients.
